Sachem Releases 2013-14 Sports Physicals Information

Sachem East High School offers free physicals this Thursday at 5 p.m.

The Sachem School District has announced its extracurricular sports and activities physical schedule for the 2013-14 academic year. All students attending Sachem High School North and East may participate in extracurricular sports including cheerleading and Arrowettes.

However, in order to participate, the following steps have been outlined by the district:

  • A physical examination card must be completed. Physicals are valid for one calendar year. If your child has a physical on record with the school nurse that was completed within the last calendar year, then a green card must be completed. Both the physical exam cards and green cards must be signed by a parent/guardian.
  • Physical exam cards and green cards are available in the nurse’s office or can be printed from our website (www.sachem.edu). Please note the physical exam card is two-sided.
  • No other forms or doctors notes will be accepted as proof of a sport physical.
  • If your child requires medication (epi pen, inhaler, diabetic, etc…), a physician’s order must be turned in with physical card or green card to be cleared to participate.
  • Sports physicals will be given at your child’s high school (at no cost) on Thursday, June 27 at 5 p.m. for Sachem North Boys and Girls entering grades 9-12. The date for Sachem East students passed on Monday.
